Fairly obviously, Ed, no on all counts. I wouldn't expect that "online
only" players are in the majority, though - not yet. In any case, I
doubt that "online only" players really care about what their NBO does
or recommends. I certainly don't feel in any way beholden to the ACBL,
for example.
I would assume that people who (for example) state that a multi 2D is
illegal are F2F players, new to online play, who are following the
regulations of their NBO. I could be wrong, of course, but in that case
I can't imagine what gives them that idea.
